Dear Parent/Guardian,
During the summer months, we will be developing class lists and student assignments for the 2022-2023 school year. If you believe that you have important information regarding your child that the school staff may not have, this form provides you with an opportunity to share that information with school personnel. Your input will be used to assist us in placing your child for the coming school year.
